Wedding Reservations

Please complete the forms below to begin the process of requesting your preferred wedding date/time. Your date is not secured, until a completed application, scheduled counseling and deposit are received. Please make all checks/money orders payable to Jars of Clay Church.  You may leave these items in the Prayer/Counseling room or mail them to:
